Backsliding…

No this is not me in high school with church attendance, this is me being a lazy ass because of a fracture.  I am the worst with both.  Maybe it has a lot based on the restrictions of both.  Although fantasies of walking and sex seem an odd comparison, unless you are me, then completely normal.

It is not that there isn’t a ton of shit going on in Medicine that needs to be repeated to the kids in the back row, it is simply that my motivation has been shot. This has been an exercise in humility and empathy for the restrictions I must put on patients. Do they maintain the boundaries like I am?  I am behaving with very little exception and those being mostly mistakes in positioning on assistive devices.  All my projects are on hold, all my fun is on hold, yep I am doing this.

Let’s talk about PrEP or Pre-exposure prophylaxis.  Recently the CDC came to 2 important conclusions the first is that maybe any sexually active person should consider PrEP.  The current drug approved is Truvada, a Gilead Pharm drug. There are others in the works, but they aren’t there yet. The chance of infection while taking correctly is minimal. Honestly if you are a man that has sex with other men, you should consider this, especially if you are not monogamous. Nothing like a call from a one night hook-up telling you he has the bug and hopes you are protected…This however my not be your only source of hope as the CDC also told us something that the WHO and the rest of us have known for 10 years.

The Centers for Disease Control and Prevention (CDC) have announced Trusted Source that they concur with scientific evidence asserting that when an individual has an undetectable HIV viral count, there’s effectively no risk of them transmitting HIV to a sexual partner.

The CDC is following the lead of more than 400 international groups that signed a consensus statement in 2016 recognizing that “undetectable = untransmittable” (U=U).

The CDC wrote, “When ART results in viral suppression, defined as less than 200 copies/ml or undetectable levels, it prevents sexual HIV transmission.”

Well, hell, it is about damn time. While I realize that the CDC has some hard working and careful people working there, the track record with HIV has sucked mainly for political reasons.  Those started under Reagan and continue to this day.  They go on to say;

“Across three different studies, including thousands of couples and many thousand acts of sex without a condom or pre-exposure prophylaxis (PrEP), no HIV transmissions to an HIV-negative partner were observed when the HIV-positive person was virally suppressed,” the CDC letter stated. “This means that people who take ART daily as prescribed and achieve and maintain an undetectable viral load have effectively no risk of sexually transmitting the virus to an HIV-negative partner.”

Cue Captain obvious.  The WHO has said this since at least 2011. Oh well I guess when it comes to gay stuff our government only likes to do it with a wide stance in an airport bathroom or with male escorts.

In all seriousness, thanks CDC for continuing to move forward, even if you are being drug into it kicking and screaming.  My feeling is that they are working overtime fighting the anti-vax bullshit.  Vaccinate your kids.  Google is full of shit.
